Accordant Health Services wants to be your health plan's best friend. The disease management company contracts with health plans and self-insured employers to offer plan members assistance in managing their chronic illnesses. Its AccordantCare service has dedicated programs for some 25 common and rare conditions from asthma and diabetes to lupus and cystic fibrosis. Accordant's case managers make sure members with those conditions stay on their treatment plans and help them avoid high-cost hospitalizations or procedures. Founded in 1994, the company is part of Caremark Pharmacy Services (formerly Caremark Rx).
